#11723 - PICU - Nurse Practitioner - Locums - 13 weeks - California (Orange, California)

Pediatric Intensive Care Nurse Practitioner needed for three month Locums contract

Start date: Nov

Duration: 3 months (possible extension depending on census)

Schedule: Two, 12hr PM shifts per week

Beds: 18 beds ICU

Patient volume: The main units the Locums NP is responsible for is 3PICU (stepdown unit) and 2PICU (High-flow oxygen and overflow unit). Both have the capacity to hold up to 12 patients, so 24 patients would be the max. 2PICU rarely has 12 patients there, and averages 6-8. However, during winter season 3PICU is typically full. If both of those units have a low census, and 6PICU is really busy with admissions, the NP should be prepared to help out with a few admissions.

All 3 units are PICU and the providers at night (attending, resident, NP, +\- fellow) are responsible for making sure all the patients are well taken care of and that the units run smoothly.

Required Procedure skill set: Comfortable with EVDs, sedation, as well as managing and adjusting vents based on blood gases and clinical picture.

Preferred Procedure skill set: Being able to perform a Lumbar Puncture would be helpful, but otherwise the procedures are optional. Typically, residents and fellows need those skills and are eager to do them. Also, if the pt is sick enough to need a chest tube, A-line, central line or intubation, those patients would be escalated to 6PICU.

Support staff: There is an in-house attending covering all three PICU units, as well as the CVICU overnight. A 2nd or 3rd year resident manages 6PICU (the more acute kids) and some nights there is a fellow overseeing 6PICU and CVICU. Each unit has a charge nurse (although 3P and 2P often have relief charge nurses who are newer to the role) and 6PICU has a trauma/code nurse who helps out with critical patients and responds to Rapid Responses and Codes throughout the hospital.

Our specialists (Endo, ID, Metabolics, Heme/Onc, Neuro, Neurosurg) are available by phone at night and there’s a general pediatric surgeon in-house for the traumas. We have a radiologist available until 11pm. After 11pm, all imaging studies can be forwarded to an on-call radiologist overnight for an official read. (The radiology tech does need to be made aware if that’s something the provider needs). In addition to a general pediatric surgeon in-house for the traumas, there is a Pediatric Anesthesiologist in house as well as a Pediatric ED MD, Neonatologist MD, and usually NICU Fellow, at CHOC-Orange and a Neonatologist at St. Joseph

Min experience: Direct pediatric intensive patient care; Clinical pediatric NP experience.
