Interim Social Services Coordinator - Senior Living

ABOUT OUR INTERIM DIVISION

ESSENTIAL JOB FUNCTIONS

  • Provides medical-related social services and coordinates the discharge process
  • Makes arrangements for obtaining needed adaptive equipment, home health services, and any other necessary community resources upon facility discharge
  • Schedules an appointment with the PCP of Skilled Nursing patients for post-discharge; makes follow up calls to discharged patients
  • Responds to a variety of resident needs, including, but not limited to, lacking or ineffective family support systems, chronic or acute medical conditions, socialization, legal or financial issues, alcohol and/or drug abuse, and an inability to cope with loss of function
  • Works closely with the Admissions Coordinator to meet the unique needs of residents and ensure they are communicated to staff and accurately documented in care plans; in the absence of the Admissions Coordinator, ensures the admissions process continues
  • Informs residents, and those they designate, about their health status, health care choices, Advance Directives/POST forms, and the discharge process
  • Secures external services from outside agencies (e.g. absentee ballots, transportation, private sitters) and assists families with Hospice referrals
  • Completes MDS interviews and Social Services assessments; provides weekly Bundled Payment Care Initiative updates on discharge plans
  • Schedules and attends SNF resident/family conferences and develops discharge care plan with input from resident/resident representative
  • Participates in weekly Standard of Care, Medicare Utilization Review, and Discharge Planning meetings; attends both SNF and Nursing Care Plan Meetings and QA Meetings
  • Supports grieving residents and secures outside support services, if necessary; communicates with Chaplain Services to ensure residents and family members/POA receive needed support
  • Serves as the facility Grievance Officer and assists with conflict resolution
  • Performs other duties as assigned

QUALIFICATIONS

Bachelor of Science in Social Work or related human services required; one (1) year of supervised work experience in a health care setting, working directly with residents and/or patients and their families, required

KNOWLEDGE, SKILLS AND ABILITIES

Ability to provide optimal care in accordance with regulations; proficiency in MS Office; ability to stand and walk for long periods and to regularly and safely lift 35 lbs.
